DS-160 ELECTRONIC VISA APPLICATION FORM
All applicants who interview for a non-immigrant visa at the U.S. Interests Section after October 1, 2011 must submit a web-based application form known as the DS-160 Non-Immigrant Visa Electronic Application

The U.S. Interests Section is offering limited appointments for applicants who wish to fill out their DS-160 at the Interests Section. To make an appointment you may:

Email havanaconsularinfo@state.gov (recommended)
- Include “DS-160” at the beginning of the subject line.
- Include your full name, your Cuban ID number and the date of your interview.
- OR -

Call the Interests Section at (+53) 7-834-4400.
The U.S. Interests Section will confirm your appointment date via the same method you use to schedule.
If you come to the Interests Section to fill out your DS-160, please ensure that you bring your current passport, along with all of the additional information listed in this Spanish-language information sheet.
DS-160 appointments are scheduled on a first-come, first-served basis two weeks in advance. Applicants who miss their DS-160 appointment must make a new appointment via one of the methods above. NOTE: These appointments are to fill out the DS-160 only. Applicants will have to return to the Interests Section for their interview(s).
